Please check the list on the left to find the form you need.  A non-refundable registration fee of $30 per child ($90 maximum per family) is due at the time of registration.  Also required is a birth certificate, baptismal certificate (if applicable) and immunization records.

Tuition Categories

Parishioner Status is determined by affiliation at either St. Francis deSales or St. Stephen Church or any of the surrounding area parishes.  To qualify, a family must be a registered, supporting member of said parish.

Non-Parishioner Status applies to those families who do not meet the qualifications set forth above.

Additional Forms

All families are required to sign a Philosophy of Volunteerism Form, obligating them to either participate in the school's various fundraising activities or make a set tax-deductible donation to the school.
