2009-2010 Registration Information

We are now accepting registrations for the 2009-2010 school year.  The following items will be required to complete your application:

  1. A copy of the birth certificate for each child you are enrolling
  2. A copy of each child's baptismal certificate (if applicable).
  3. A $100 per family non-refundable registration fee (please make checks payable to Immaculate Conception School). 
  4. The New Student Registration Form (see registration forms)
  5. Tuition, Fundraising and Childcare Agreement (see registration forms)
  6. SMART Tuition Agreement Form (available through the school office)
  7. Health History Questionnaire (see registration forms)
  8. Developmental Questionnaire Form (see registration forms)
  9. Request for Bus Transportation Form (see registration forms, or contact the bus garage directly).  A written request for bus transportation must be completed and received by the bus garage/transportation department of the local school district before April 1, 2009.
  10. A copy of your 2008 Federal Income Tax return showing AGI (if you qualify to pay less than the "Over $100,000" rate on the tuition rate schedule)
  11. Family/Church Commitment Form, signed by your pastor (if you qualify to use the "Families with Church Subsidy" as noted on the tuition rate schedule)
  12. Completed Emergency Form (see registration forms)
  13. A New York state physician verification of immunizations.
  14. Proof of physical exam done in the last 12 months by a New York State licensed physician, physician's assistant, or nurse practitioner.
  15. Students enrolling in Pre-K, Kindergarten or Grade 1 are required to present a dental health certificate; such dental health certificate must contain a report of a comprehensive dental examination performed on such child.
