2009-2010 Registration Information

We are now accepting registrations for the 2009-2010 school year. The following items will be required to complete your application: - A copy of the birth certificate for each child you are enrolling
- A copy of each child's baptismal certificate (if applicable).
- A $100 per family non-refundable registration fee (please make checks payable to Immaculate Conception School).
- The New Student Registration Form (see registration forms)
- Tuition, Fundraising and Childcare Agreement (see registration forms)
- SMART Tuition Agreement Form (available through the school office)
- Health History Questionnaire (see registration forms)
- Developmental Questionnaire Form (see registration forms)
- Request for Bus Transportation Form (see registration forms, or contact the bus garage directly). A written request for bus transportation must be completed and received by the bus garage/transportation department of the local school district before April 1, 2009.
- A copy of your 2008 Federal Income Tax return showing AGI (if you qualify to pay less than the "Over $100,000" rate on the tuition rate schedule)
- Family/Church Commitment Form, signed by your pastor (if you qualify to use the "Families with Church Subsidy" as noted on the tuition rate schedule)
- Completed Emergency Form (see registration forms)
- A New York state physician verification of immunizations.
- Proof of physical exam done in the last 12 months by a New York State licensed physician, physician's assistant, or nurse practitioner.
- Students enrolling in Pre-K, Kindergarten or Grade 1 are required to present a dental health certificate; such dental health certificate must contain a report of a comprehensive dental examination performed on such child.
